(3) Send at least $100 deposit. (Balance Due upon notification of completion)
(4) Phone number, e-mail address, and shipping address
(5) If name is desired on helmet, please write legibly
(6) List ideas that you like (Flames, Checkerboard, Scallops, Carbon Fiber, etc.)

The reason for this is that not everyone likes flames, checkerboards, etc. I will use the above information to give you the best design I can come up with.
